As we look forward to re-opening our school, we want to ensure it is a safe and welcoming environment for all our pupils.

We kindly ask that this form is completed for EACH child in our school. This ensures that we all take personal responsibility to keep our pupils and school community safe.

If you answer ‘YES’ to any of the questions, please seek medical advice before allowing your child to return to school. Upon receipt of medical advice, please contact the school Principal before your child starts with us.
